Instructions for Completing IT-QEE-TP1
In the top section, “Enter for Contributor,” you will enter your personal information and that of your spouse if you are filing jointly. Enter your social security number in the space entitled “Taxpayer Identification Number” if you are an individual or couple filing jointly. If you are filing as a C corporation, enter your Tax ID number.
In the space entitled “Tax year end for contributor” enter the end of the current year, i.e. December 31st, 2009 (or may be the end of a fiscal year for a corporation). In the space entitled “Calendar year in which contribution is made,” enter the same calendar year, i.e. 2009.
On page 2, in the space entitled “Taxpayer Identification Number,” enter your social security number for individuals or your Tax ID number for corporations.
Be sure to print and sign your name.
Do not fill out anything in section D.
Send your form to the address listed in section C (just above the beginning of section D).
